Do You Qualify?

Five questions, about a minute. It will not tell you that you qualify, and nobody but a certified Florida doctor can, but it will tell you whether the appointment is worth booking.

Cancer, epilepsy, glaucoma, HIV, AIDS, PTSD, ALS, Crohn's disease, Parkinson's disease, multiple sclerosis, chronic nonmalignant pain, and a terminal condition diagnosed by a second physician. A qualified physician may also certify a condition of the same kind or class as those listed. A physician makes that judgment; the published list itself does not grow, so conditions people often assume qualify (anxiety, for example) are not named in the statute.
The state charges $75 for the Medical Marijuana Use Registry ID card, payable every year, and it is the same wherever in Florida you apply. Paying online adds a $2.75 convenience fee, which makes it $77.75 that way; a check or money order is $75. Physician certification is separate and is set by the clinic and the state has no part in it; we do not publish clinic prices because they vary and change, so ask when you book. The card renews annually while your certification runs its own cycle of at most 210 days, so expect more than one certification a year.
Not a medical opinion

This quiz is an informational tool. It gives no diagnosis and is not a determination of eligibility, and not medical or legal advice. Only a Florida-certified doctor can decide whether you qualify, and the Office of Medical Marijuana Use issues the card, not us.
